I hope you’re doing well. I’m applying to [program/scholarship/job], and I wanted to ask if you would be willing to write a strong recommendation letter for me.
I took your [course name] in [term], and I really valued [specific thing from class]. I especially enjoyed working on [paper/project/topic], and I think your perspective on my [writing/research/class participation/etc.] would be really helpful.
The deadline is [date]. I’ve attached my resume, transcript, and a short summary of what I’m applying for in case that’s useful. I’m also happy to send anything else that would help.
If your schedule is too full, I completely understand.

A few extra things that helped:

Make the subject line clear. Something like Recommendation letter request - [Your Name].
Be specific about the opportunity. Don’t just say “I’m applying to some programs.” Say what kind and why.
Send a small brag sheet. Mine had 5 bullet points: grades, projects, leadership, goals, and anything I hoped they might mention.
Waive your right to view the letter if the application gives that option, unless you have a reason not to. A lot of recommenders expect this.
Follow up politely. I sent one reminder a week before the deadline and another two days before only if needed.
Say thank you after. I sent a thank-you email right after they submitted, then updated them later when I got results.
